Issue:
You want to add a widget using a Notes form as a Notes form rather than a web page. The Add Wizard for widgets has a View entry but not one for a form or frameset. If also says that if you have the view, form, or frameset open, you can create a widget automatically using the widget toolbox icon that appears (if you have the toolbar active) or you can do it off the Menu of Options icon on the My Widgets bar in the sidebar. However, if you preview the form or open via actions or button, no such context exists.

Workaround:
You can open up a frameset and the context sensitive "feature" works sort-of. Once you have the form open:
Choose Tools -->
Widgets --> Configure a widget from the current context.
You can then edit the URL line and change the frameset listed to the form you want instead and it will work.

Also, notice the path is really a Notes URL:
Notes://joshua2/<db rep id>/<design element>?OpenForm

Technically, at this point, you can change it to whatever you want.
